Report an Issue with an IT Service

To report an issue with and IT service, follow the link here and complete the form. Both the Requested For box and the Department box should auto-populate your information. If they do not, start typing your name/department and it should search for the correct information. 

In the Subject box, type the service that is having issues. For example;  "I cannot log in to the wireless network" or " I cannot print from my workstation" or "I cannot download software form the software center"

In the Description box, describe the issue or problem you are having. Please be as specific as possible.  For example; I am tying to connect to the wireless network from my laptop computer. I am in Butte Hall 405 and I get an error message saying 'failed to connect.  Or "When I attempt to print to my office printer, I don't see it as an option in the drop down menu as I have before." or "Every time I try to down load Zoom from the software center from my office desktop computer in MLIB 338, all I see is the spinning wheel and the message saying 'download failed'"
